Myriad Genetics: On Day One of JP Morgan Healthcare Conference, San Francisco, January 12, 2015 – January 15, 2015

Myriad Genetics CEO Peter Meldrum provided a broad overview of the company’s test portfolio, starting with myRisk Hereditary Cancer, a panel of 25 genes for evaluating a patient’s risk for colorectal, breast, endometrial, ovarian, prostate, pancreatic, and gastric cancer, as well as melanoma.

According to Meldrum, conversion to myRisk from the firm’s BRACAnalysis test has gone well enough that Myriad’s laboratory capacity was recently overwhelmed, resulting in longer turnaround times for results, though the issues have been resolved.

He also noted that myRisk has been able to find 51 percent more people at risk for hereditary breast cancer than BRACAnalysis.

In October, Myriad received a draft local coverage determination from Medicare contractor Palmetto GBA for its Prolaris prostate cancer test, and Meldrum said on Monday that the company anticipates a national coverage decision by Medicare to come, though he didn’t provide a timeframe.

Myriad has also been active in pursuing companion diagnostic test development deals, and in late 2014 the US Food and Drug Administration approved BRACAnalysis CDx as a companion diagnostic test for AstraZeneca’s Lynparza (olaparib) for treating advanced ovarian cancer associated with BRCA mutations.

Lynparza is a poly ADP-ribose polymerase (PARP) inhibitor. Myriad has companion diagnostic development partnerships with five pharmaceutical firms that are also developing PARP inhibitors, Meldrum said. He added that in the US and Europe about 400,000 patients could use such drugs, and companion diagnostics to guide treatment, each year.

In total, Myriad is collaborating with more than 20 pharmas in the companion diagnostic space, he said, noting that in addition to PARP inhibitors, the firm is developing companion diagnostics for platinum drugs used as adjuvant and neoadjuvant therapies.

Regarding Myriad’s international business, the company now has a direct sales force of 45, Meldrum said, and in fiscal year 2014, its international revenues grew 109 percent year over year. For FY 2015, the company has targeted $50 million in revenues for Europe.

Meldrum also said that in Europe, the firm may adopt a strategy of offering its tests as kits that can be used in reference laboratories, rather than having tests carried out in Myriad’s lab. Such a step would have the potential to grow revenues in Europe, where the use of reference labs, instead of a company-owned lab, is prevalent, he noted.

Illumina Launches Four New Systems; Provides Financial, Dx Update at JP Morgan

SAN FRANCISCO (GenomeWeb) – Illumina today announced the launch of three new sequencing systems and provided an update on its diagnostic plans in the noninvasive prenatal testing and oncology markets.

Speaking at the JP Morgan Healthcare Conference, CEO Jay Flatley preannounced fourth quarter revenues of $512 million, as well as $1.86 billion in FY 2015 revenues, representing 31 percent year-over-year growth. He said the firm expects revenue growth of 20 percent in FY 2015.

He also unveiled the HiSeq X Five System, the HiSeq 3000 and 4000 Systems, and the NextSeq 550 System, an NGS system enabled for array scanning.

The HiSeq X Five is a scaled down version of the X Ten, which was launched at last year’s JP Morgan conference. It consists of five individual HiSeq X instruments, enabling more “capital-constrained” customers to adopt high-throughput whole-genome sequencing with a lower capital investment, Flatley said. The X Five provides the throughput to sequence more than 9,000 genomes a year, he noted, and will cost $6 million per system. Customers will be able to produce a genome for around $1,400 with the system, which will launch in the second quarter of 2015.

Flatley said that the firm sold far more X Ten systems in 2014 than it had initially anticipated. In total, he said the firm signed up 18 customers and sold 201 X Ten units during the year, including 37 sold in the fourth quarter.

The HiSeq 3000 and 4000 systems build on the capabilities of the HiSeq 2500 System and leverage the patterned flow cell technology developed for the HiSeq X Ten System.

Roche to Invest up to $1.2B in Foundation Medicine, Take Majority Stake

NEW YORK (GenomeWeb) – Roche plans to take a majority stake worth more than $1 billion in oncology genomic testing firm Foundation Medicine, and will invest potentially more than $150 million in R&D, the companies said today. In addition, Roche will help to expand sales of Foundation’s tests outside the US.

Roche will commence a tender offer for approximately 15.6 million shares of Foundation Medicine at $50 per share, for an aggregate value of about $780 million. The offer price is a 109 percent premium over Foundation Medicine’s closing price last Friday.

In addition, Roche plans to invest $250 million in Foundation Medicine through new shares, issued at the same price, to fund operations and development.

Combining the tender offer and direct investment, Roche would own between 52.4 percent and 56.3 percent of Foundation Medicine on a fully diluted basis.

Under an R&D collaboration agreement, Roche also plans to fund research activities with more than $150 million over at least five years, and to contribute its expertise in oncology.

Foundation Medicine will continue to operate independently, and will be led by current President and CEO Michael Pellini and his management team. Also, Foundation’s existing pharmaceutical business will not be impacted by the deal.

“By combining [Foundation Medicine’s] pioneering approach to genomics and molecular information with Roche’s expertise in the field of oncology, we can bring personalized healthcare in oncology to the next level,” said Daniel O’Day, chief operating officer of Roche Pharma, in a statement.

“The structure of our agreement with Roche also allows us to maintain the entrepreneurial spirit at Foundation Medicine and ensures that our business model, network of partnerships, and objectives are not altered,” Pellini said.

The R&D collaboration will initially focus on genomic profile tests for cancer immunotherapies and continuous blood-based monitoring. Roche plans to use Foundation’s platform to standardize clinical trial testing in order to compare results for R&D and, eventually, for the clinic.

The collaboration and resulting tests are expected to support the development of combination therapies, novel targets, more accurate patient stratification for clinical trials, and new companion diagnostics.

In addition, Roche and Foundation Medicine signed a commercial collaboration agreement, under which Roche obtains rights to existing products outside the US under Foundation’s brand, as well as future co-developed products. In the US, Roche plans to engage its medical education team to provide information to pathologists.

Foundation Medicine currently offers two clinical assays: FoundationOne for solid tumors and FoundationOne Heme for hematologic malignancies, sarcomas, and pediatric cancers.

Both the R&D and the commercial agreement will become effective once Roche’s direct investment in Foundation Medicine and its tender offer are completed. The investments and collaborations are subject to approval by Foundation’s shareholders and customary closing conditions.

The companies’ boards of directors unanimously approved the transaction and three Foundation shareholders owning a combined 31 percent of the firm’s equity – Third Rock Ventures, Kleiner Perkins Caufield & Byers, and Google Ventures – are supporting the deal.

The transaction is expected to close in the second quarter. Upon closing, Foundation’s board of directors will increase to nine individuals, including three determined by Roche, among them Daniel O’Day. Foundation CEO Michael Pellini will remain an independent company director, along with five others. Alexis Borisy is expected to remain chairman of the board.

Roche is obtaining financial advice for the deal from Citi and legal counsel from Davis Polk & Wardwell. Goldman Sachs is Foundation Medicine’s financial advisor, and Goodwin Procter is the firm’s legal counsel.

Separately today, Foundation announced preliminary total revenue of approximately $18.7 million for the fourth quarter and $61.1 million for full-year 2014 — a 93 percent and 111 percent increase from $9.7 million and $29.0 million recorded in Q4 and full-year 2013, respectively.

Foundation reported 7,233 clinical tests to ordering physicians in Q4, compared to 3,752 tests in Q4 2013. The company reported a total of 24,271 FoundationOne clinical tests to ordering physicians for full-year 2014, a 167 percent increase over the 9,095 tests reported in 2013.

Overall Themes and Scope of PMWC2015 – January 26-28, 2015

Personalized Medicine World Conference 2015 (PMWC2015) is the 7th annual gathering of key opinion leaders and all stakeholders who constitute the personalized medicine field—researchers, industry participants, clinicians, regulators, payors, and emerging companies in this space, making it “a must attend event”. Highlights range from research-related topics such as Next Gen Sequencing (NGS) and its clinical utilization and prenatal Diagnostics to regulatory and commercially focused presentations. This conference brings together all the key decision-makes in personalized medicine for three days of intensive scientific exchange, partnering activities as well as networking opportunities. PMWC2015 will highlight the following topics:

The Research Arm of Personalized Medicine

The strong interest in prenatal and newborn diagnostics is being addressed in an opening lecture by Dennis Lo, Li Ka Shing Institute, followed by presentations by companies that operate in this space. Another key topic area well addressed at PMWC2015 is the deployment of NGS addressed by practitioners such as Illumina, Thermo Scientific and Trovagene, that offer tools and technologies allowing the deployment of NGS into clinical practice. Content-focused companies such as Foundation Medicine frame the opportunity for cancer care through NGS. Genomic profiling in routine clinical care enabled by companion diagnostics is discussed by Amgen’s Scott Patterson, along with speakers from Abbott and Thermo. PMWC brings to light the emerging themes in the migration of NGS toward the clinic. The adoption of Personalized Medicine is being driven by the pervasiveness of NGS. It is estimated that 50% of academic oncologists currently refer their patient samples for NGS-based genomics testing—of course this is only a small fraction of the total patient population since a vast majority of cancer patients receive care in community hospitals without access to NGS and other molecular medicine tools. Frank Ong, Associate Director of Medical Affairs at Illumina, chairs a session focusing on the impact of NGS in clinical care beyond cancer as it is gaining traction in many other therapeutic areas. Indeed, NGS is bigger and far-reaching beyond merely oncology. In the cardiovascular area the discovery and development of biomarkers for more precise diagnosis and treatment is the focus of a session chaired by Gil Omenn, from the University of Michigan. The streamlining of clinical trials through the utilization of personalized medicine to stratify and target responder patient populations is addressed at PMWC, highlighting the advantage of cost-effectiveness to pharmaceutical companies and clinical value to patients and payers.

 

Regulatory and Reimbursement Trends in Personalized Medicine

PMWC2015 offers a forum for the US FDA Commissioner, Margaret Hamburg to comment on evolving FDA regulation on laboratory developed tests (LDTs) – key area of change in this industry and of paramount importance to personalized medicine. George Lundberg from CollabRx chairs an important session on ethics and public law in personalized medicine—an area gaining interest as personalized medicine begins to permeates all aspects of the practice of medicine.   Without reimbursement personalized medicine will not prevail. Palmetto GBA will give an update on the evolving reimbursement trends in personalized medicine. The regulatory and commercial framework of personalized medicine is absolutely critical to its deployment and acceptance industry-wide, and PMWC brings together the FDA and legal experts such as Hank Greely from Stanford to comment on the ethical and legal framework surrounding patient privacy and confidentiality, as well as moral perspectives of personalized medicine.
